Does anyone have an example of a signature sheet that they can send me. Our code official is requiring a signature sheet for the project manual that we are submitting for permit.

Check with the registration board of the state where the project is located. Some require a seal, most require specific text. Also, some require wet signatures, while other, more enlightened states, allow use of an image.
Some also require specific configurations, dictating the location of the seal in relation to the signature. “Enlightened” states? What’s that? Those in sunshine?
Yep, all that stuff. It’s made more interesting by local agencies and building officials, who often have their own interpretations of what is required.
We are fortunate to have a single board of registration here, so the certification rules are the same for everyone. Another state we have worked in has one board for architects, another for engineers, and maybe more. No surprise, but the text, configuration, and rules for use of each certification differ.

Our seals page (or pages) has simple boxes, two columns wide, with the discipline at the top of each box and the PE/Arch’s name at the bottom of the box. Like drawings, many jurisdictions want to see the wet seal, so we circulate a few copies to the PEs to stamp. Fortunately for us, that is usually no more that a walk downstairs.
Arizona is “enlightened” in both aspects: they allow digital seals and it is currently sunny and 72 degrees (Phoenix metro area, that is).
And what was it last July?
It was a nippy 115 degrees.

Has anyone encountered a requirement for electronic signature pages? If so, how did you resolve? How are electronic signatures authenticated/verified?
Are drawings electronically stamped/signed? I have been “out-of-the-loop” re: drawing production since the advent of CADD, so I am somewhat clueless about current technology/capabilities.
By “electronic signature” do you mean a graphic image or a digital signature?
As I understand it digital signatures requires adding some specially encrypted text to the file of the complete document. Any additions or changes to the file would void the digital signature. I see problems if you try to get multiple individuals to apply their digital signatures to the document.
First the digital signatures would need to be applied to a file of the complete project manual which means that this would be the last thing done.
You would have each individual apply his digital signature to the file in sequence.
I would suspect that many consultants would not have the ability to apply a digital signature. It is not clear how somebody else can apply a digital signature for you.
I am assuming that you are doing work in a state that allows the use of digital signature.
Where am I wrong?
A graphic image does not have any authenticity, as I can easily scan/insert an image of seals, stamps, signatures, etc. So I guess that what I am inquiring about are digital signatures. By Mark’s comment, it is apparent that multiple digital signatures may be problematic. After some more online research, it appears that digital signatures, depending on the level of authenticity desired, could require third party certification.
So, back to my original question, has anyone encountered any requirement for digital signatures…for any level of authenticity/certification? If so, how was it accomplished?
